Outcome Support Services and The Eden Initiative work is greatly needed in many communities. Outcome Support Services strategy and strenght is based on family and being a family run organization. We provide goal based solutions for families in need of extra care and assistance with their loved ones.
We help by providing:
- Community Support Companions, to plan outings and provide supportive assistance when needed. Increasing the independence of the persons we support is one of our goals, as is community participation, self-care and safety skills.
- Behavioural Intervention, to children, adolescents, adults and seniors with developmental delays, including autism spectrum disorders, dimentia, alzheimer's and other similar behavioural and mental diabilities.
- Goal-Oriented Respite Care, to increase independence of the persons we support
- Crisis Prevention and Emergency Behaviour Management/ Positive Behaviour Support Planning for children, adolescents and adults demonstrating severe problem behaviour.
We subscribe to the belief that all behaviour has meaning. Challenging behaviour is seen to serve a very important function for the learner, and until that function is addressed the behaviour will not improve. In addition to reactive/emergency behaviour management strategies, our positive behaviour support plans include preventative strategies (so that the problem behaviour is not needed in the first place).
All of our Support Plans have individualized, operationally defined behavioural objectives. At any given moment in our service delivery, we know exactly what the goals are, how to attain them, and our criteria for behaviour change. Our data collection procedures and automated charting allows us to determine the level of progress towards our goals at any given moment. Through baseline measures and data tracking and analysis over time, we can measure our outcomes.
